-trans/genderqueer/intersex can mean very different things, so I'd not list them all as one option (Some people will also get offended to be listed along side one of the others, or argue if these belong under questions about sex instead of gender, or under both, etc). If possible, I think its better to just say "What's your Gender? [___________] (and fill in the blank)".
